Applications are available at the PMHA office located at 176 Rustic Drive, Circleville, Ohio 43113 Monday through Friday 8:00 am – 4:00 pm.

When submitting the completed application, the applicant must bring the following documentation:

  • Verification of Income for ALL family members who receive income. Include name and address of income sources.
  • Birth Certificate for ALL family members.
  • Social Security Card for ALL Family members.
  • Name and Address of bank(s) & value of assets.
  • Green Card and Passport for ALL family members who are not United States citizens.

Note: Head of Household must be 18 years of age or emancipated at the time of application. Any balance owed to PHMA or any Housing Authority must be paid in full before housing assistance will be offered.

Public Housing Application Information

The application is a written document. If you are unable to complete the application, a PMHA employee will assist you. Applicants must provide the following information to determine eligibility:

  • Names of all persons who will be living in the rental unit, the sex of those persons, their date of birth, and their relationship to the family head of household
  • The present address and telephone number of the applicant family
  • Indication of veteran status and other family characteristics that may qualify the family for tenant selection preferences
  • Names and address of current and previous landlords
  • Information on the family's anticipated income and the sources of that income
  • The names and addresses of employers, banks and any other income sources that the Housing Authority would need in order to verify income and deductions, and to verify family composition
Additional Documentation

If not presented at the time of application, the Public Housing representative may request documentation needed to verify the information given on the application. We will also rely on direct verification from employers and other sources. At the Intake Appointment, all household members 18 years of age or older are asked to sign a form to authorize release of the pertinent information needed to determine eligibility and to process the application.

Notification to Applicants

All applicants are notified of additional documentation needed at the time the application is submitted. All applicants are also notified concerning the eligibility status of the application and whether the application has been placed on the waiting list or denied. All eligible applications are placed on the waiting list. Once an application nears the top of the waiting list, the applicant is contacted to schedule an Intake Appointment.

Waiting List Preference Points

Applicants are placed on the waiting list according to points given for veteran status as well as date and time of application. We offer the preference to U.S. military veterans and the spouses or widows (not divorced) of U.S. military veterans if the veteran had a minimum of 6 months active duty and a honorable discharge. Veteran’s Preference is also extended to those who qualify for veteran benefits, with the veteran supplying documentation from the VA.

Public Housing

Veterans are awarded 1 point. This is given to veterans or spouses of veterans. Veteran status is verified by the DD214 form.

Housing Choice Voucher

Veterans are awarded 1 point. This is given to veterans or spouses of veterans. Veteran status is verified by the DD214 form.

Williamsport Terrace

No preferences are awarded. Applicants are placed on the waiting list by date and time of application.

Pickaway Terrace, Eden Place, and Louise Terrace

Veterans do receive preference on these waiting lists. All applications are placed on the waiting list by date and time of application, with veterans placed above non-veterans according to the date and time of the veteran application.
